To qualify for Professional Pricing with discounts as much as 20% - 50% please complete the following.

1. If you have not previously created a F.O.G. Cosmetics store accounts please do so here.
2. E-mail or fax one of the following:
• Hair or Makeup Union Card
• Cosmetology or Esthetician License
• Tear Sheet with credit
• An on-line portfolio link with makeup images
